All association coaches, board members or volunteers that have any contact with players:

If you are a coach, board member or volunteer that has any contact with players from an Association within MWAHA, you are required to fill out an online Consent to Screen form.

It should only take about few minutes. Failure to comply with this request will result in your removal from any contact with players or association with the program as a whole.
