Coaching Checklist and Application

1) USA Hockey & MidWest Affilate Forms

Please download, print, complete and return the following forms to the Omaha Hockey Club office by either:

A.) Mailing your completed documents to:

B.) Dropping off your completed documents at the Omaha Hockey Club mail slot at the Moylan Tranquility Ice Plex.

C.) Scanning your completed documents and emailing them to:

2) USA Hockey On-Ice Coach Registration

Before applying to be a head or an assistant coaching position, you will need to register as an on-ice Player / Coach by clicking here.

This registration will cover you as a player if you still play in addition to coaching. Remember to print out the confirmation page (with barcode) and return it to the Omaha Hockey Club office (

3) Omaha Hockey Club Coaching Application form.

Please take the time to complete all fields and submit.

Contact the Director for your division to inquire if an application is required.

4) USA Hockey CEP Certification

Your last step to becoming an Omaha Hockey Club coach is to obtain your proper USA Hockey coaching card (certification).  Do this by signing up for and completing coaching certification clinics. Coaches have until December 31st of each season to obtain their credentials.

5) Consent to Screen Form (Online) Background Check

All coaches, volunteers, and board members: 

If you are a coach, volunteer, or board member for an Association within MWAHA, you are required to fill out an online Consent To Screen form.  It should only take about 5 minutes.

6) USA Hockey Safesport

All coaches, volunteers and board members are required to take the USA Hockey Safesport training.  For anyone that may have contact with the players in or around the locker room or on the ice they must complete this training before doing so.  Please click on the link and complete the FREE online training course.

7) Concussion Certificate (Online)

All coaches are required to complete the concussion certificate by November 1 of each year and provide a copy to the Omaha Hockey Club Risk Manager, Age division director and age group commissioner.
