Injury & Concussion Reporting and Resources

If a player sustains an injury during any OHC event—whether on or off the ice—it must be reported using the official USAH Injury Reporting Form. This includes games, practices, team activities, or travel associated with USA Hockey. Promptly completing this form ensures proper documentation, helps protect all involved, and supports any necessary follow-up.


An "injury" is defined as: Any ice hockey related ailment, occurring on the rink or player’s bench, that kept (or would have kept) a player out of practice or competition for 24 hours or required medical attention (trainer, nurse or doctor) and all concussions, lacerations (cuts), dental, eye and nerve injuries.


If a player is injured during a team activity and subsequently visits a doctor for evaluation—regardless of how serious the injury is or seems—they may not return to any physical team activity without a written doctor’s note (no emails) explicitly stating the player is "cleared to participate in ice hockey." There are no exceptions to this rule.


For players who have sustained a concussion, please have the Concussion RTP Form filled out and signed by a medical professional to begin any OHC activity.
